COVID-19 Update

Although the Governor has moved Florida into Phase 3 of re-opening, our office will continue to be closed to all "Walk-In" traffic due to the small size of our lobby that makes social distancing a challenge.  We have made many enhancements to our Website, and many forms can be submitted from your cell phone or computer. In addition, we have a drop box located at our front door that is checked throughout each day.  We are continuing to conduct Annual Re-Exams by mail.  We do understand that there are some times when you may need to come into the office.  Your case worker will determine if an appointment is needed and  can schedule an appointment with you.
